Specific Directions:
* Take the BETHEL RD. exit off SR-315.
* Go WEST on BETHEL RD.
* Pass through the traffic light at KENNY RD.
* Look for the Third (3rd) break in the median strip and turn LEFT on (DALMENY CT.)
After turning on Dalmeny Ct. (Follow the ARROW on the MAP BELOW)
*Dalmeny Ct. will become a circle. Turn LEFT on the circle.
* Follow the circle around to the right
Then look for the building containing 5100 (South Eastern corner of the complex)
Please park in one of the two (2) assigned spaces marked 5100 painted on the asphalt.

LESS than a 24hr. notice of cancellation by the therapist will result in a 50% discount off the missed appointment when rescheduled.
NO SHOW - Failure to appear for a scheduled appointment withiout prior contact will result in a 100% charge of the scheduled appointment price.
